Recall and Administration Care Coordinator

Main Duties of the Job
This is mainly an administrative role to monitor and administer the practice recall systems, actively inviting patients for their long-term condition reviews and other appointments as required. The successful candidate will need to have excellent attention to detail, good IT skills, work well both in a team and under their own initiative, and have a positive can-do attitude.

Our patient list size is currently around 18,000.

Job Responsibilities
The following are the core responsibilities of the Care Coordinator for Recalls and Administration. There may be occasional requirements to carry out other tasks; this will depend upon factors such as workload and staffing levels:
1. Full management of the long-term conditions reporting
2. Maintain a good working knowledge of current QOF and recall processes and requirements
3. Administer the patient invitation process for long-term condition reviews and other appointments as required
4. Monitor patient recall and recall systems
5. Ensure accurate report output
6. Deal with queries via email, telephone, and SystmOne tasks relating to Long Term Conditions patients and recalls
7. Ensure patient records are updated accordingly
8. Liaise with the practice clinical team
9. Liaise with patients, relatives, hospitals, and other agencies
Person Specification
Experience
* Experience of working with the general public
* Experience of administrative duties
* Experience of working in a healthcare setting
Qualifications
* Educated to GCSE level or equivalent
* GCSE Mathematics & English (C or above)
* NVQ Level 2 in Health and Social Care
