ABOUT THE HOSPICE OF WINDSOR AND ESSEX COUNTY INC.
Founded to provide compassionate, clinical, end-of-life care, The Hospice of Windsor and Essex County Inc. (Hospice) has been caring for our community since 1979. Hospice believes in equitable access to care for all. We provide specialized wrap-around services to adults, children, and their caregivers, in the setting of their choice – whether it be in our Residences or in their home.
With only partial government funding, it has been through the generous support of our volunteers and our community that we have continued to provide memorable care, meeting the physical, emotional, and spiritual needs of patients and their loved ones.
ROLE OVERVIEW
This role will appeal to a compassionate individual who values hospice palliative care and who is eager to support our Hospice organization and a collaborative team. The Social Worker is dedicated to the values, mission, and vision of the Hospice of Windsor and Essex County within the organization and the community, and puts their best efforts forward to contribute to a positive work culture.
The Social Worker is experienced and knowledgeable about life-altering/threatening diagnoses and the broader field of palliative care, with special emphasis on end-of-life care. The Social Worker manages a caseload of patients as well as runs psychosocial group programs at locations as assigned by the Manager of Psychosocial Services, demonstrating professional values and compassion for patients, families and visitors. They develop and promote relationships within the community with all health partners.
PRIMARY DUTIES AND RESPONSIBILITIES
1. Receive referrals from the Manager Psychosocial Services or Clinical Admin Assistants and respond to clients within response guidelines;
2. Provide individual and family counselling; visits are conducted virtually, by phone, or in-person at Hospice or client homes throughout Windsor and Essex County;
3. Provide support to individuals and families at the Hospice Residences (Windsor, with flexibility to provide coverage in Erie Shores);
4. Develop and facilitate group programs;
5. Complete psychosocial assessments, develop goals and related plan of care in collaboration with the patient and caregivers, and make recommendations regarding appropriate social work intervention;
6. As part of an Interdisciplinary team, design and implement group counselling and programs that address common psychosocial needs of patients and caregivers;
7. Communicate, implement and ensure that Hospice’s responsive and holistic culture is maintained;
8. Develop and nurture professional/therapeutic relationships with community partners;
9. Communicate and implement any of the Hospice’s support services as appropriate for families and patients;
10. Within the Hospice organization, assess for psychosocial education needs and assist in providing education to staff and volunteers;
11. Participate in team meetings for case consultation and information sharing;
12. Complete electronic scheduling and documentation for assigned patients and complete ongoing updates and monthly statistical reports, while prioritizing patient needs;
13. Attend meetings and conferences for the purpose of coordinating services and/or professional development;
14. Assist in supervision of University Social Work Interns and other students of related programs as deemed beneficial in the progression, promotion, and development of Hospice support services and community palliative care;
15. Other responsibilities that may be assigned by Leadership.

ACCOUNTABILITIES
1. Accountable to the Manager of Psychosocial Services. Case reviews are completed with the Manager of Psychosocial Services for ongoing performance evaluation;
2. Accountable for commitment to Hospice Mission, Vision and Values;
3. Accountable to maintain all Hospice policies and professional behavior and for ensuring review as required/indicated;
4. Accountable to maintain confidentiality and act in an ethical and professional manner consistent with the professional code of ethics and agency policies;
5. Accountable to maintain practice requirements through the annual renewal of the professional college certification, and exhibiting clinical competence through continuing education/protocols as established by the Ontario College of Social Workers;
6. Accountable to maintain performance in accordance with the palliative care competencies for social workers as defined by the Ontario Palliative Care Competency Framework (Reference: The Ontario Palliative Care Competency Framework: A Reference Guide for Health Professionals and Volunteers, April 2019, pp126-137).
REQUIRED QUALIFICATIONS AND EXPERIENCE
1. TECHNICAL REQUIREMENTS
o Current registration in good standing with the Ontario College of Social Workers and Social Service Workers and proof of annual renewal by December 31st of each year;
o Able to perform as an individual counsellor, as well as provide group development and facilitation of programs;
o Excellent oral, written and telephone communication skills;
o Computer proficiency and experience with electronic documentation systems, spreadsheets and/or database systems is preferred;
o Must possess a car and hold a current valid drivers’ license;
o Willingness to work a non-traditional and flexible schedule;
o Demonstrated ability to work in a collaborative team environment;
o Bilingual French/English an asset.
2. EXPERIENCE
o Experience with grief and bereavement through all patient populations is preferred;
o Experience working with children and adolescents is considered an asset;
o Five (5) years work experience in a social work role is preferred.
3. EDUCATION
o Master of Social Work Degree from a recognized University program.
