An exciting opportunity has arisen for a highly motivated individual to join our Adult Service Team.
You will be working with Adults aged 16 years and older who might have a range of health and social care needs. Your core work will be underpinned by the principles of Self Directed Support and Social Work Scotland Act. You will be working under the supervision of Operations managers and will be responsible for the completion of a holistic assessment of risks and needs, developing support plans to meet identified needs.
You will also be responsible for reviewing the assessed outcomes of individuals and how the support plan is meeting their needs, maximising their independence, and ensuring is delivered in a cost-effective way. A key aspect of this role is working with multi-disciplinary colleagues within South Lanarkshire Health and Social Care Partnership, and the ability to promote and encourage a collaborative working approach with Adults, partner agencies and voluntary organisations is essential.
Our Social Workers are also required to support people within a legal framework; this includes working with the Adults with Incapacity (Scotland) Act, Mental Health (Care and Treatment) (Scotland) Act and the Adult Support and Protection (Scotland) Act for social workers with Council Officer training. This statutory work allows Social Workers to work in more depth with supported people, build meaningful relationship and support them to make safer choices with positive results.
We would welcome applications from registered Social Workers and from Final Year Degree in Social Work Students looking to start their career as a Social Worker. Final year students must be qualified within 3 months of submitting their application form.
