Job summary The post holder will be the Lead Specialist Midwife for Clinical Governance and Risk Management for maternity and gynaecology services and will take the lead role in incident reviews and analyses; lead on serious incident reviews and reports; clinical audit, clinical effectiveness and MDT mandatory education and will also be the Trust Patient Safety specialist for maternity & gynaecology. The post holder will present and receive complex, sensitive information which may require analysis and interpretation in order to inform the clinical governance plans for maternity and gynaecology. The post holder will be responsible for the day to day oversight of all clinical incidents, conducting patient safety reviews and incident investigations in line with the Trusts PSIRF (Patient Safety Incident Response Framework) policy and the oversight of the risk register. The post holder should have a sound understanding of the principles of maternity governance and PSIRF and experience of investigating concerns /incidents. Hours per week: 37.5 Main duties of the job Work with Head Midwifery/Women's Children's services, the Clinical Lead and Operational Managers to maintain a clinical governance and risk management framework that fosters a culture and system of ownership and accountability, whilst encouraging innovation and problem solving. With the Head of Midwifery, take the responsibility for the completion of the Risk Register for maternity & gynaecology services and the escalation of risks to the Planned Care directorate and upwards to the Trust Board. To monitor the development and implementation of evidence-based guidelines and protocols in response to local and national documents. Collate and monitor all reported incidents analysing for trends and produce regular reports for the service, directorate, wider Trust and external stakeholders as required To ensure that reported clinical incidents are investigated and analysed and the lessons learned disseminated though established networks, internally and externally. To be responsible for the notification of Serious Incidents in maternity & gynaecology upwards to the Trust Board. To identify training needs highlighted through the risk management process, provide leadership to the Practice Development Midwife to ensure provision of training on maternity critical skills in line with MIS and the annual Training Needs Analysis reflects local requirements.
