Description Your role as a Youth Access Worker As a Youth Access Worker, your role is to support young people by understanding their unique needs, challenges, and strengths, conduct thorough assessments to identify risks and opportunities, create personalised support plans, and connect youth to the resources they need to thrive. Building trust and fostering positive relationships are central to the role, ensuring young people feel heard, valued, and empowered to navigate life's challenges. Through collaboration with families, community services, and multidisciplinary teams, this role will provide holistic care that helps young people achieve stability and long-term success. This is a short term 6-month contract with possibility of an extension. Key accountabilities include Comprehensive Assessments: Conduct detailed assessments to identify young people's needs, risks and strengths Relationship Building : Build trust, respectful relationships with young people to foster engagement. Collaboration and Coordination : Work with multidisciplinary teams to deliver holistic care. Case Management and Documentation : Maintain accurate and up-to-date case notes, assessment and reports ensuring documentation complies with organisation and legal requirements. Ability to work collaboratively with young people and within a multidisciplinary team. Ability or understanding to conduct assessments on young people, touching on sensitive topics and assessing needs. Experience, or willingness to learn, to develop and deliver group programs. Knowledge of mental health as it relates to young people - the clinical presentations, and its stressors and triggers. Understanding of clinical risk and other risk factors in mental health relating to young people. Eligibility/other requirements Successful completion of a National Criminal Check. Successful completion of an NSW Working with Children Check. Valid New South Wales Driver’s Licence.
