We are a successful and highly regarded GP training practice based in the beautiful coastal town of Scarborough on the North Yorkshire Coast. With a patient list size of 15,900 patients, Scarborough Medical Group operates from two surgeries to the North and South of the town.

Main duties of the job
We aim to ensure that the highest levels of care are delivered to all our patients, particularly in areas such as minor illnesses and acute, on the day appointments.
The main duties of the role are to assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions relating to minor illness and within the boundaries of professional qualifications.
The post holder will use advanced clinical skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment.

Job Summary
The Clinical Practitioner willbe responsible for providing routine and acute medical services to patients whowish to be seen by ahealth care professional. You will assess, diagnose, treat, and write prescriptions as required.You will triage and refer patients appropriately, making any necessaryreferrals for investigations in the appropriate manner including to SecondaryCare.
Primaryresponsibilities
The following are the coreresponsibilities of the Clinical Practitioner.
You will work within your own scope of clinicalpractice to:
18. Assess patients presenting with acuteillness.
19. Diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis within the boundaries of competence.
20. Evaluatepatients' medical conditions and determining the appropriate medications toprescribe. The individual will be responsible for writing prescriptions,ensuring the correct dosage and instructions, and monitoring patients'responses to the medications.
21. Clinically examine and assesspatients needs from a physiological and psychological perspective, and planclinical care accordingly.
22. To undertake home visits, primarilyfor acute conditions, as assessed by the duty doctor, and within the boundariesof professional qualifications.
23. Instigate necessary invasive andnon-invasive diagnostic tests or investigations and interpret findings/reports.
24. Assess, diagnose, plan, implement andevaluate interventions/treatments for patients with complex needs or liaise on patient care with the most appropriate memberof the clinical team as required.
25. Proactively identify, diagnose andmanage treatment plans for patients at risk of developing a long-termcondition.
26. Diagnose and manage both acute andchronic conditions, integrating both drug and non-drug based treatment methodsinto a management plan.
27. Ensurecontinuity of care, arranging follow-up consultations or reviews as necessary
28. Supportpatients in the use of their prescribed medicines or over-the-counter medicines(within own scope of practice)
29. Liaisewith external services/agencies to ensure that the patient is supportedappropriately (vulnerable patients, etc.
30. Ensurethat they adhere to the relevant patient group directives and local clinicalpathways at all times
31. Maintainaccurate clinical records in line with legislation, ensuring read codes areused correctly
32. Fully participate in the clinicaladministration within the practice, signing prescriptions, reviewingmedication, reviewing patient letters/discharges, responding to queries.
33. Prescribe and review medication fortherapeutic effectiveness, appropriate to patient needs and in accordance withevidence based practice and national and practice protocols and formularies.
34. Work with patients and their carers inorder to support compliance with and adherence to prescribed treatments.
35. Provide information and advice onprescribed or over-the-counter medication on medication regimes, side-effectsand interactions.
36. Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care.
37. Support patients to adopt healthpromotion strategies that promote healthy lifestyles, and apply principles ofself-care.
38. Support and manage the needs ofpatients presenting for family planning or sexual health consultations,including the prescribing of contraceptive medication or LARC.
39. Assess, identify and refer patientspresenting with mental health needs in accordance with the NSF for MentalHealth.
40. Contribute to the practice achievingits quality targets to sustain the high standards of patient care and servicedelivery.
41. Help develop and set up new patientservices and participate in initiatives to improve existing patient services.
42. Helpdevelop and review clinical protocols.
43. Attendthe review of significant and near-miss events meetings ensuring learnings andactions are undertaken and followed up within the Practice
