Health Care Support Worker (End of Life)
We are seeking compassionate and motivated Healthcare Support Workers to join our Swindon End of Life Care Team (SELeCT). This team is dedicated to providing exceptional end of life care, supporting patients who wish to spend their final days at home.
Your role will be crucial in facilitating rapid hospital discharges, ensuring that both emotional and personal care needs are met for these patients. If you have a passion for end of life care and want to make a meaningful difference, we invite you to become a part of this team.
Main Duties of the Job
You must be able to demonstrate a real enthusiasm for community nursing and particularly end of life care. You must also have appropriate or transferable skills as well as a desire for ongoing professional development. Shifts will include evenings and weekends. You must be keen to learn and adapt to a changing environment, as well as being expected to complete the Care Certificate training if you have not already obtained this qualification.
Record keeping is electronic, so you must be prepared to learn how to use our systems. You will require excellent communication skills, have a kind and caring nature, and be able to show empathy to the patient and their family at a time when they will almost certainly be feeling anxious and distressed. A driving licence and access to a vehicle is essential for this role.

Job Responsibilities
Our service ensures end-of-life patients are able to be cared for in their own homes, 365 days of the year. This role is within the Swindon End of Life Care Team (SELeCT), part of the multi-professional community nursing team, providing holistic support, personal care, and treatment to individuals who choose to die at home.
Person Specification
Qualifications
* NVQ Level 3 in Health Care - or willingness to achieve.
* Care Certificate - or willingness to achieve.
* Food Hygiene Certificate - or willingness to achieve.
Experience
* Previous experience in providing personal care is essential.
* Experience of supporting other workers.
* Able to work independently under the delegation of the Registered Nurse.
* Experience of the NHS.
* Experience of community working.
Knowledge & Skills
* Ability to work as a team member.
* Delivering patient-focused care.
* Ability to recognise own limitations.
* Physical skills to manually handle patients and use appropriate lifting aids.
* Basic wound care.
* Experience of Aseptic Non-Touch Techniques and an awareness of infection control procedures.
