What you will do:
1. Lead and oversee the Adults Community Team, ensuring the delivery of high-quality, person-centred services.
2. Manage daily operations, coordinating care and support for adults with various health and social care needs in the community.
3. Provide leadership and guidance to a multidisciplinary team, promoting effective collaboration and communication.
4. Develop and implement best practice policies and procedures to improve service provision.
5. Monitor and evaluate service delivery, ensuring compliance with all relevant regulations and standards.
6. Engage with service users to understand their needs and expectations, fostering an environment that supports individual empowerment and choice.
7. Collaborate with other stakeholders, such as healthcare professionals, social services, and community organisations, to enhance service delivery.
