Join the Home First Therapy Team in Buckinghamshire
An exciting opportunity has arisen to become part of the Home First Therapy team in Buckinghamshire. As the team is still developing into a substantive service, team members will have the chance to influence and shape its growth.
We are seeking motivated and skilled individuals to support patients recently discharged from hospital, helping them regain independence and remain in their own homes. The ideal candidate will adopt a holistic, person-centred approach to support recovery from hospital admission and help achieve short-term goals to maximize independence.
Role Responsibilities
1. Visit patients at home to perform assessments and treatments for those with complex needs within expected timescales.
2. Provide advice, guidance, and information to health and social care professionals, carers, relatives, and other non-professionals.
3. Perform all aspects of clinical duties as an autonomous practitioner.
4. Participate in professional and service development and contribute to organizational objectives.
