To work as the identified Early Help Liaison practitioner to deliver consultation, advice and information sharing for community services and schools working with children and young people who may present with mental health difficulties. Co-ordination and implementation of evidence based targeted interventions in a timely manner with the emphasis on early intervention to deliver best outcomes for CYP. To ensure that children and young people access timely assessments and the appropriate targeted interventions. Have the knowledge of external services and be able to signpost children, families & professionals. Providing 15 Clinical contacts with young people and families each week. To support the delivery of evidence based group programmes for children and young people with mental health issues from a portfolio of available CAMHS training. Take a lead in representing CAMHS in multi-agency meetings such as EHAT, SEND, CIN meetings or child protection meetings to co-ordinate and have oversight of mental health element of a multi-agency plan around a child. To function as a member of the CAMHS Early Help Pathway taking appropriate responsibility for the implementation of decisions made by the pathway. Ensure planned interventions are implemented effectively and evaluated using Routine Outcome Monitoring (ROMS) and minimum data set. Maintain standards of care to highest level within available resources. To actively participate in line management with senior and junior members of staff. To liaise effectively with the other CAMHS pathways. To develop and maintain relationships with providers statutory and non-statutory. To maintain confidentiality in respect of the young people at all times. Communicate clearly with children and young people, their families and all other health professional groups. Actively participate in clinical governance and quality improvement initiatives, e.g. clinical audit, essence of care or clinical policy development. Ensure that children and young people are encouraged to play a part in decision making about the help they need and how they can access specific interventions. This is to promote person-centred planning. Provide mentorship for students.The post holder will be responsible for updating their own practice, knowledge and skills within their sphere of practice including Livewell Southwest mandatory training. Excellent knowledge of the principles of the CYP IAPT programme and a willingness to undertake training. Expert ability in receiving information, analysing and formulating professional judgments. The post holder will provide assessment and targeted interventions focused on Early Help working within the I Thrive model of care to offer the most appropriate input to the children and young peoples needs. Ability to match children and young peoples need with available skills and resources.To take a lead role in the co-ordination of allocation of work with the available skill mix utilising, prioritising, negotiation and delegation.Liaise with other services to arrange appointments that both meet the needs of child or young person and service demands.Confidence to travel in an urban environment.Competent IT skills.Take a lead in early help assessments. Formulate plans for appropriate interventions or to sign post to relevant services. Making appropriate agreed referrals for longer-term treatment and attending CAMHS core multi-disciplinary meeting to discuss onward steps to ensure CYP is on correct pathway. Take a lead role in clinical decision-making and collaborating with other team members in the provision of children and young peoples care packages. Ensure the on-going effective use of routine outcome monitoring in line with CAMHS Early Help Pathway.Take a lead role in the identification of risk and the formulation of risk management plans within the community setting. Provide education to children, young people and their families to enhance their knowledge therefore improving longer-term concordance. Promote resilience and prevention of mental health issues. Maintain high standards of clinical care to children, young people and their families. To liaise and work with other clinicians within CAMHS Early Help Pathway to ensure the needs of children and met by the right team, on the right pathway at the right time.Expected to contribute to the development of the CAMHS early help pathway policies including the formulation of protocols. Implement all appropriate National, Livewell Southwest and Local Home Treatment policies and protocols. Provide clinical supervision and line management supervision for junior members of staff. Have the ability to challenge staff where the standard falls below expectations. Assist in education programmes provided for junior staff. Provide supervision for students both formally and informally as part of the supervisor/assessor. Participate regularly in recruitment and selection of staff. The post holder will behave in a manner that does not bring the Team or Livewell Southwest into disrepute.Responsible for Systm1 contacts and monitor the standard of work of junior staff. Maintain accurate, timely paper and electronic records. Ensure they are kept safely in accordance with Livewell Southwest policy.Provide a positive role in clinical audit and service evaluation to ensure high levels of clinical effectiveness. The post holder will be individually accountable for their practice in accordance with the NMC professional Code of Conduct or HCPC.The post holder will actively participate in line management supervision with the Clinical Pathway Manager of the Early Help Pathway.Provide line management for junior staff. The post holder will actively participate in clinical supervision in accordance with Livewell Southwest policy. The post holder will actively participate in the Livewell Southwest appraisal system and provide appraisals for junior staff.
