Job summary
Following highly successful pilot, the Trust seeks to appoint a committed and suitably experienced specialist end of life nurse, who will consolidate existing service provision, developing an expanded service offer. The post holder will be expected to design, coordinate and lead a team who will provide outstanding end of life care in the Emergency Department (ED), in addition to working with pathways into and out of the ED. The post holder will be a beacon of good practice, setting standards and supporting other members of the team, and wider MDT, in 'getting it right first time'.
The post holder will be supported to extend the current service offer in providing a 7-day professionals advice line (pre-conveyance support) and where absolutely essential, peripatetic function. There are a number of offsite community beds which the Trust utilise for end of life and supportive care, which the post holder and their team will provide in-reach clinical support. This role will form part of the Supportive and Palliative Care Service senior clinical leadership team at UHP, and as such, the post holder will be expected to support other clinical area teams (such as the in-hospital team) when required. This unique role represents a fantastic opportunity to lead and expand a vanguard service, which has made significant improvements to end of life care in the Trust over the last 18 months.
Main duties of the job
1. Focus on the delivery and maintenance of clinical excellence in End of Life care in ED/ED pathways
2. Provide an expert consultancy service to colleagues including pathways into/out of the ED in relation to remote advice and support
3. Plan, implement and evaluate evidence-based service delivery across areas of responsibility
4. Provide support (clinical and operational) across the End of Life service line teams as part of senior UHP End of Life leadership function.
5. Be accountable for KPIs and deliverable outcomes in relation to Trust initiatives/End of Life transformation plans relating to role; escalate to Trust End of Life lead where agreed KPIs are not being met and jointly plan in meeting these
6. Contribute to the development of the service by taking an active role in generating and disseminating knowledge/providing education across the organisation/partner organisation
7. Ensure the service to patients/families focuses on improving outcomes and experiences for EoL patients, families and carer

As an expert senior nurse in the field of End of Life and palliative care, the post holder will practice autonomously at a specialist level in thedelivery of high quality, safe and effective service, including roleblends a significant proportion of direct, higher level clinical care with strategic leadership and management. The post holder will work with patients, families and carers, providing direct nursing care and clinical advice to the multidisciplinary team. The post holder will work autonomously, leading a team of professionals; the postholder will work closely with Trust initiatives and End of Life projects/transformation plans, aligning service delivery with Trust KPIs, in tandem with patient need
To ensure your team facilitates better patient/carer experience of dying within the ED; where appropriate, to facilitate timely transfers of care to appropriate locations (community and bedded locations), in line with patient choice (preferred place of care and preferred place of death)
To actively coordinate and contribute to team efforts in facilitating specialist knowledge transfer by providing ad-hoc (working alongside colleagues) and planned education events/opportunities, in and outside of the ED (partner teams, internally and externally)
Be part of the clinical leadership team within the UHP Supportive and Palliative Care Service (across all elements of the service, as required). The post holder will represent the Trust and present, provide reports, reporting on project deliverables, and be accountable for service KPIs.
Work closely, aligning service offer, with partner teams, internally and externally, including acute oncology, departments relating to urgent and emergency care, Virtual Ward, AGP (as examples) and with external stakeholders, such as primary care, St Lukes Hospice, care coordination hubs, Livewell and SWASFT (as examples)
To coordinate and co-deliver peripatetic function in relation to admission/conveyance avoidance; this will include on-site support of offsite UHP End of Life bed bases
Design, coordinate and support the delivery of a 7-day service professionals pre-conveyance telephone/virtual support advice service.
