As Specialty Manager, you are part of our newly formed Integrated Frailty Leadership Team; together with a Clinical Director, AHP lead and Matron, you lead the SBU and are a key member of the division's management structure, leading and driving our Integrated Frailty Services, including:o Single Point of Accesso Hospital at Homeo Urgent Community Responseo Intermediate Careo Milford Integrated Care HubYou will also be working with services across the whole Frailty pathway into an acute setting, including:o Frailty Same Day Emergency Careo Acute Frailty Teamo Inpatient wards at both acute and community hospital sites Main duties of the job Your role will include:o Leading service delivery across a challenging yet rewarding community setting.o Supporting and inspiring multidisciplinary teams working across the frailty pathway.o Driving large-scale service transformation projects in partnership with the leadership team to deliver against strategic objectives outlined in the Frailty Strategy.o Monitoring and managing planned and unplanned activity, ensuring budgetary control.
