Working across NWAFT to offer a specialist TYA Cancer service provision to those patients 13-24 years with a cancer diagnosis. The role would lead on the operational and day to day coordination of the Teenage and Young Adult Cancer Service (TYA) at Peterborough City Hospital. Working in collaboration with the Principal Treatment Centre (PTC; Cambridge) Teenage Cancer Trust Lead Nurse and the TYA MDT to ensure all patients (13-24 years) receive age appropriate high quality clinical care. Working in collaboration to ensure they receive age appropriate high quality clinical care and support. Acting as a source of advice and clinical expertise, providing a comprehensive specialist role for TYA with cancer to improve both outcomes and experience. Working alongside the existing site specific teams (adult services) and Paediatric Oncology / Haematology teams, the post holder will provide advice on age appropriate care and support to patients eligible for the TYA Service in line with the national guidance and regional operational policy. Main duties of the job To promote and facilitate a continuous supportive cancer service across Hospital and Community. To be a supportive member of the cancer multi-disciplinary team (MDT). To act as a resource to other health care professionals, who are involved in the management of cancer. To have a continuing responsibility for the provision of specialist information and support for a caseload of TYA patients across NWAFT. To participate in service delivery ensuring that it meets national recommendations and directives for patients with cancer.
