Job overview
The successful post holder will be an autonomous practitioner undertaking assessment, consultation and delivering management of care to children within the Children’s Community Nursing Team (CCNT) caseload. You will be working within the CCNT with a small caseload of patients as well as receiving referrals and supporting the CCNT to prevent children and young people from requiring admission to hospital.
Working alongside the Paediatric Virtual Ward team to allow children and young people to receive care from home, you will work closely with the Paediatric Emergency Department, Paediatric Assessment Unit (PAU), the Children’s Inpatient Ward (Rudham), and GP practices to support care delivery in the community where appropriate.
You will be expected to work within the four pillars of Advanced Clinical Practice; supporting both the nursing and medical team providing care to children and young people with the primary focus of keeping children at home.
Although predominately based in the CCNT, there is an expectation that you will cross cover PAU and work alongside other Advanced Clinical Practitioners on the ward area.
Main duties of the job
1. To work autonomously, managing own caseload of patients to assess, plan, implement and evaluate care.
2. To provide specialist advice and support to children and young people (CYP) and their families to improve self-management of their condition within the community setting.
3. To appropriately visit, triage and support patients who may be unwell, undertaking a physical examination and recognising deviations from the CYP’s normal status.
4. To recognise clinical presentations of a sick child or young person.
5. Maintain a high standard of nursing care applying evidence-based practice.
6. Assess, plan, implement and evaluate child/family centred care considering individual ethnic, cultural, social, developmental and religious needs.
7. Coordinate pathways of care for children and young people on the CCNT caseload.
8. Maintain accurate records.
9. Update patient administration systems to ensure real-time data collection.
10. To maintain confidentiality of information (written, verbal, electronic), whilst being aware of the Freedom of Information Act.
11. Communicate effectively with children and young people, their relatives/carers, significant others, the multi-disciplinary team and support services.
12. Lead & support the participation of clinical audit or reviews of patient care, as required.
Work with all clinical teams within paediatrics to support children and young people to receive timely care regardless of location.

Person specification
Qualifications/training and professional development
* RN (Child) or RSCN
* Educated to Degree level (Masters)
* Advanced Nurse Practitioner with recent paediatric experience or Emergency Nurse Practitioner with recent paediatric experience. Evidence of working to the 4 pillars of advanced practice.
* Non-medical prescribing qualification or willingness to undertake.
* Teaching and Assessing/Mentorship qualification.
* EPLS/EPALS.
* Clinical Assessment Module completion (e.g. PACS).
Experience
* Worked as a band 6 or above within a paediatric setting.
* Extensive experience of assessment, planning, diagnosis and implementation of care to children and young people.
* Clinical Governance experience.
* Experience of managing change in clinical practice.
* Experience of working in the community.
* Experience in leading a team or in routinely coordinating a shift in an acute paediatric setting.
Skills, abilities and knowledge
* Understanding of chronic disease management.
* Ability to deliver expert nursing care to children and young people.
* Evidence of multi-professional team working.
* Confident and calm attitude with ability to work under pressure.
* Accurate documentation with attention to detail.
