This role calls for someone with exceptional interpersonal and communication skills, a knack for motivating others, and an understanding of the diverse challenges faced by individuals with Dementia and their families. Your mission? To reach out to Dementia individuals across Swansea and Neath, offering helpful advice, info, practical support, and personalised guidance through a holistic approach. By developing and implementing person-centred carers plans, you'll empower them to navigate their journey with confidence. Are you up for the challenge? A day in the life of a Community Connector: Develop, collaborate with and maintain positive and effective working relationships with partnership agencies throughout the community Engage with service users to identify specific needs and appropriate interventions Provide high quality information, signposting and promotion to a wide range of services Support people to attend a local community service suitable to their needs Work with individuals newly diagnosed with Dementia and their families What does it take to be a successful Community Connector? Ability to work as part of a team and on own initiative Knowledge of the issues surrounding Dementia care. Ability to plan your own workload Understanding of how to improve service quality for the benefit of users Willingness to work flexible hours if needed Full clean driving licence, with a minimum of 2 years driving experience and access to a vehicle.
