Work in a variety of settings as allocated by the Acute Liaison nurse. Provide support to people with learning disabilities, their carers and families including face to face working as allocated by Physical Health Liaison Practitioner. Support may include virtual and telephone contacts when planning appointments. Gather relevant information to draft a reasonable adjustment care plan under the direction of the acute liaison nurses. Attend the learning disability MDT hub meetings as required. Gather information for individuals referred to the Acute Liaison Service including supporting information gathering for the Learning Disability Bundle, risk assessments, care plans under the direction of a registered practitioner. Liaise with ULHT, community health care services hospitals and GP practices to actively support people with learning disabilities to access mainstream primary and secondary acute health services. Undertake training as required to meet service requirements. Provide a high standard of clinical care in ones own practice and the teams practice. Work flexibly to meet service need. Coordinate, and prioritise own workload under the direction of the Acute Liaison Nurse Undertake delegated aspects of the role whilst maintaining responsibility for their own clinical caseload. To be responsible for keeping caseload reviewed and updated liaising with Acute Liaison nurse and Safeguarding Lead The post holder will receive clinical and management supervision, a yearly appraisal and mandatory training in line with LPFTs policies. Assist in obtaining valid patient consent and work within a legal framework with patients who lack capacity to consent to treatment. Protect people from abuse, neglect and harm and raise concerns to ULHT/LPFT safeguarding as appropriate. Work within LPFT and ULHT agreed policies and procedures, protocols, promote best practice and. ensure adherence to Safeguarding policies and procedures across services. Understand the implications of the Mental Capacity Act and acts to assess capacity as appropriate. You may occasionally be asked to undertake such other duties that are deemed necessary in order to support business continuity within the Trust.
