Key Responsibilities Work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up to date records. To carry out assessment and treatment of cardiac patients from inpatient stay through to full rehabilitation in the community with support and guidance from senior colleagues. Provide specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required. To support the Team in the day to day running of the Cardiac Rehabilitation Service To support and advise within the wider MDT on the delivery of the service To work within the Cardiac Rehabilitation service, developing effective clinical interventions within the speciality in order to contribute to clinical practice in the team in line with current evidence base. To manage autonomously a clinical workload of patients / clients assessing and treating patients and maintaining clinical records with the support of senior colleagues. To have an understanding of the assessment and treatment skills required in the areas listed but not limited to: Specialist knowledge of all cardiac conditions Cardiac function including physiological testing Exercise physiology Physical rehabilitation through exercise and function Psychosocial effects particular in relation to stress and anxiety Motivational strategies Education and health promotion relating to all cardiac conditions To apply evidence based knowledge within the specialist clinical area acquired through experience and academic study. To act as a resource and provide advice for other team members, other professionals and colleagues within the MDT about the assessment, treatment, management and care of patients / clients. To work collaboratively with health and social care community and acute colleagues. To work with other members of the MDT to ensure effective care pathways and transfer of care / discharges. To proactively and appropriately facilitate a seamless transfer of care and continuing therapeutic management of cardiac patients between care settings. To work collaboratively in an interdisciplinary manner within the MDT, and where competent take on aspects of blurred boundary working, where appropriate. To assist in the development of specialist programmes of assessment, treatment and intervention for cardiac rehabilitation patients in both hospital and community settings as appropriate. To manage and prioritise own clinical workload as appropriate, working efficiently to manage professional time to ensure the effective delivery of the service. To develop and actively undertake clinical audit as requested. Maintain full, clear and timely patient records in accordance with Departmental and Trust policy.
