Job summary
An excellent opportunity has arisen for a enthusiastic and ideally experienced, Care Navigator with the ability to work well under pressure to join our friendly practice team with 2 GP partners and 10,200 patients.
Shift patterns are Mon - Fri between 8 - pm and you will be required to work from either of our 2 sites. We are accepting applications for Full or Part time postions.
Main duties of the job
To be responsible for undertaking a wide range of reception, secretarial and administrative duties and the provision of administration support to the multidisciplinary team. Duties can include but are not limited to, greeting and directing patients, effective use of the appointment system, booking appointments, admin triage, prescriptions, registrations and deductions, processing of information (electronic and hard copy) in a timely manner and assisting patients as required.
To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers, in accordance with current polices.
