We are looking for a band 5 therapist to join our Long Term Conditions team. You will join a team of SLTs working with people with communication and swallowing difficulties due to neurological and respiratory conditions, frailty and at end of life. Care is provided in outpatient clinics, patient’s homes, care homes and community wards. This is a county wide post and it is essential you can drive.
This is a brilliant early career opportunity, you will have access to CPD courses, support from a team who are passionate about high quality community care, and exposure to a wide range of conditions that impact communication and swallowing. As a team we have low waiting times enabling us to offer timely support and intervention to our clients.
You will be supported in this role. Colleagues are available for case discussions and joint working, as well as regular clinical supervision and peer support. The post holder will be supported to develop autonomy in their work at their pace, and to develop skills with rehab interventions, dysphagia competencies and newly qualified therapist competencies.

We welcome applications from students who are due to qualify in the next 4-6 months, and who may be able to start in post as a band 4 pending their HCPC registration.
The post holder will be a member of a team of therapists working in community and hospital settings in Buckinghamshire. They will work alongside senior therapists and have access to support. The post holder will work within the community adult speech and language therapy team as part of the long term conditions service. They will provide assessment, diagnosis and treatment of people presenting with communication and swallowing difficulties, offering care in people’s homes, care homes and outpatient locations across the county. The post holder will be required to travel throughout Buckinghamshire. They will be accountable for their own actions, recognising professional boundaries and competencies.
· To manage a defined caseload and work with an agreed degree of autonomy and independence, with regular support/supervision from the speech and language therapy team.
· To identify, assess and make a differential diagnosis of communication and swallowing impairments and select appropriate patients and type of therapy for effective treatment.
· To develop clinical skills and professional knowledge through experience, training and supervision and to work towards developing competencies in agreed areas of patient management.
· To take part in the supervision of students with support.
· To take part in supervision of the work of SLT assistants and volunteers when required.
· To take part in the development and delivery of training to others.
