Job summary
An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a Registered Nurse. The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients.
Our team delivers excellent patient focused care to patients in the East Suffolk community. Could you be a valued key member of our team? We will offer an individually tailored induction, training and support for you.
The role involves travelling throughout East Suffolk delivering acute care in patient's home as a part of REACT nursing team, both during core hours and out of hours. You will be working in an internal rotation. You will be expected to work early shifts, late and night shifts including weekends and bank holidays. During out of hours between 18:00-8:00 the Nurse will be accompanied with a Health Care Assistant. Pool cars are accessible for staff to use and it is essential that you have a valid driving licence.
Main duties of the job
1. To assess, plan and deliver healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
2. To work with the multidisciplinary team delivering Outpatient Parental Antibiotic Therapy (OPAT) in the community to facilitate discharge from hospital or prevent unnecessary admission to hospital
3. To work within the integrated team to prevent unnecessary admission to hospital.
4. To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
