Job summary The Treating Tobacco Dependency have an exciting fixed term opportunity to join them, to work, support and contribute to the implementation and management of the tobacco dependent treatment service and the smoke free initiative at Greater Manchester Mental Health NHS Trust. Tobacco Dependency Treatment Practitioner will support the day-to-day running of the Tobacco Dependency Treatment Service to ensure safe, well organised, and evidence-based care is offered routinely to all identified smokers and provided to all that access the service. This role will be diverse with opportunity to visit several sites across the GMMH footprint to deliver smoking cessation interventions and support the Trust's smoke free initiative. Tobacco Dependency Treatment Practitioner will make a valuable contribution to improving health and reducing health inequality amongst people with SMI. Main duties of the job Deliver tobacco harm reduction (smoking cessation) assessments, advice and support sessions including the provision of motivational and behavioural support. - Contribute to the development of individual Tobacco Dependence care plans - Support and advise patients with SMI to switch to and access NRT whilst in mental health inpatient. - Be a positive role model to each ward-based smoke free champion and other clinical staff in supporting patients to reduce harm from tobacco whilst maintaining a strong visible clinical presence - To support the implementation of the Smokefree Policy and help increase confidence in Harm reduction/ Stop Smoking conversations. - Ensure robust discharge planning for the patient's ongoing tobacco dependency treatment for patients wishing to quit or who have an ongoing harm reduction programme - Line manage Tobacco Dependency Treatment Advisors assigned - Countersign notes About us Greater Manchester Mental Health (GMMH) Foundation Trust employs over7000members of staff, who deliver services from more than 160 locations.
