We are seeking a compassionate and organised individual to join our Hospice Rapid Access Service (HRAS) as a Care Coordinator. This role is vital in ensuring that our patients receive the highest quality of care by coordinating and managing their healthcare plans.
Location:Basildon Hospice
Salary:£17,010 per year (full time equivalent: £24,301 per year)
Hours of work:Part-time, 26.25 hours per week
Contract:Permanent
Key Responsibilities for Care Coordinator
The primary duties of the Care Coordinator include:
* Developing and managing comprehensive care plans for patients.
* Coordinating with healthcare professionals to ensure continuous and consistent patient care.
* Monitoring patient progress and adjusting care plans as necessary.
* Providing support and guidance to patients and their families regarding healthcare needs and services.
* Maintaining accurate and up-to-date patient records.
* Organising and scheduling appointments, treatments, and follow-ups.
* Facilitating communication between patients, families, and healthcare teams.
* Ensuring compliance with all healthcare regulations and standards.
Qualifications and Experience Required
We are looking for candidates who demonstrate the following:
* Previous experience in a healthcare or care coordination role.
* Strong organisational and multitasking abilities.
* Excellent communication and interpersonal skills.
* Proficiency in Microsoft Office applications (Word, Excel, Outlook).
* Ability to work both independently and as part of a team.
* Knowledge of healthcare regulations and best practices (desirable).
