Your new companyBirmingham City Council are looking to recruit a number of Qualified Social Workers for their Discharge to Access (D2A) Service.
Your new roleBirmingham City Council are looking for an experienced social worker to work within our adult services provision, essentially required to support with facilitating care packages for patients ready for discharge / in the process of being discharged from hospital, and both services work closely together (D2A refer patients to EICT, when required).You will also have extensive knowledge of social work practice and legislation, including the Care Act 2014 and Mental Capacity Act 2005, a good understanding of personalisation and excellent assessment and analysis skills.This role will pay depending on experience, on an ongoing contract. JD is available upon request.Working Hours: 8:30am-5pm (Mon-Thurs) and 8:30am - 4:30pm (Friday).Hybrid Working: 2 days in office per week.
