Entry of patient information on Lorenzo, Assist in ordering stock for Virtual Ward. Liaison with Consultants/Senior Nurses and their Teams. Issuing guidance to patients regarding ward pathways or protocols Ensuring all clinicians receive results promptly. Participate in the daily handover and meetings. Supervise, support and help registered nurses in the development and orientation of other Health Care Assistants Have a responsibility for the tidiness and promoting cleanliness of all patient areas, including equipment rooms and storerooms, and take appropriate care of furniture and equipment. Ordering and maintaining stock control of appropriate patient information and shared care agreements. Education and Training Implement audits as delegated by clinicians to monitor. Participate in the Trusts appraisal system. Maintaining own professional and personal development and up to date knowledge of research in relation to this area of work together with relevant government policies and strategies Raise any training requirements. Professional Ensure that the Ward Manager is informed of any professional concerns at the earliest opportunity, Always behave in a professional manner and adhere to the Trust policies. Special Requirements The post holder will be expected to frequently exert moderate physical effort for short periods on a shift when undertaking clinical duties and should expect to be frequently exposed to unpleasant conditions, e.g. housing conditions, body fluids, e.g. faeces, vomit, blood, urine. when working clinically in the community and ward areas. The post holder will be frequently exposed to distressing / emotional situations during the course of a shift whilst delivering patient care to patients homes, or ward areas. The post holder will be expected to be able to manage and prioritise work load when there will be competing demands for their time and attention. The post holder will be required to have access to a reliable vehicle, insured for business use, in order to facilitate travel for scheduled home visits.
