The Crisis Resolution and Home Treatment Team for older adults is a well-established, friendly and supportive multi-disciplinary team based in Peterborough. Covering the Peterborough, Huntingdon and Fenland areas, the team offers intensive support and treatment at home, and in care homes, for older adults with mental health challenges, including the behavioural and psychological symptoms of dementia.
The service we offer includes:
* Crisis intervention, intensive treatment, support, risk assessment and management to prevent unnecessary hospital admissions and facilitate earlier discharge from mental health hospital.
* Dementia Intensive Support which offers a practical and psycho-educational approach to care within patients' homes and care homes.

Main duties of the job
* The post holder will work within the Crisis Resolution and Home Treatment Team for older people to provide a mental health service seven days a week, within a community setting, by providing a psychosocial assessment to those who present with a range of mental health problems.
* To work in partnership with service users and carers to provide treatment within the least restrictive environment.
* To take on the role of care coordinator for clients and provide leadership structure support to others, ensuring that full assessment of individual care needs are undertaken, that care is planned, implemented and reviewed on a regular basis.
* Provide support and guidance to the band 6 members of the team to aid in care planning and treatment options/pathways for functional patients within the crisis team service.
* To provide assessment to those at risk of being admitted to hospital and provide care, support and treatment within a community setting or facilitate admission to hospital and promote rapid discharge.
* To provide clinical risk and mental health assessment, formulation and short-term contingency planning for people presenting in mental health crisis.
* To act as a role model and promote effective teamwork across the service to meet individual client needs.
* To provide support for the team lead, assist with supervision and training within the team as required.
* To provide a crisis and home treatment service to patients, which incorporates dementia intensive support.

Job responsibilities
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. * Dementia Nurse Specialist role will sit within the Crisis Resolution and Home Treatment Team for Older People. The post holder will be responsible for developing and implementing evidence-based strategies within the Dementia Intensive Support (DIS) service.
* To support the delivery of the Dementia Intensive Support Service to people living with dementia, experiencing a high level of behavioural and psychological symptoms and associated health needs.
* The post holder will be required to work in a variety of locations such as nursing/residential homes, the person's own home and undertake educative activities and psychosocial interventions with a wide range of stakeholders.
* To provide clinical leadership to colleagues in the CRHTT to ensure the delivery of person-centred and evidence-based care.
* To contribute to and undertake evaluation of the DIS, in relation to clinical and service outcomes.
* Supporting the Team Leader and Service Manager in the operational management of the DIS is a fundamental component of this role.
Person Specification
Education/Qualifications
* Degree or equivalent level of experience.
* Non-medical prescriber or willingness to undertake.
Experience
* Experience of working with people living with dementia and carers in a variety of settings.
* Demonstrable experience of working as an autonomous practitioner.
* Demonstrable leadership skills.
* Enhanced training in the Mental Capacity Act.
* Previous experience of undertaking Continuing Care Assessments.
* Experience of working with older adults and people living with multimorbidity and frailty.
Skills & Abilities
* Able to demonstrate commitment to high quality care and service provision.
* Decision making, delegation and problem-solving skills.
* Able to deliver a range of therapeutic interventions.
* Proven clinical assessment skills.
* Able to use research methods and methodologies.
* Knowledge of dementia and the effects on a person's life and the impact on carers and families.
* Knowledge of theory and application of person-centred dementia care.
* Thorough understanding of care planning and the Care Programme Approach (where appropriate).
* Able to design behaviour management strategies which are rooted in evidence-based practice.
Physical Requirements
* Physically able to tolerate long periods of intense concentration.
* Physically able to tolerate frequent car journeys/travel between sites.
