Are you looking for a new challenge? How about caring for Hospital patients in their own home?
Hospital@Home service is looking to recruit an enthusiastic and experienced registered nurse to join their team based at Broomfield Hospital.
This is a highly visible role providing strong professional and clinical leadership within the Hospital@Home (H@H) team. The successful candidate will assist with supporting the sub-acutely unwell patient by providing care in the patient's home, administering IV medications, wound care management, supporting admission avoidance, triage and management of the patient in the community setting.
We also manage our Hospital@Home clinics where we will see the same cohort of patients who are more mobile, within our (OPAT) clinic on the hospital site and the successful candidate will have opportunities to work within the clinic setting as well as in the patient's own home.

If you enjoy holistic one-to-one care, with the time to really support your patients, then this is the job for you.
The role is predominantly based in the community (although there will be some hospital site working), therefore you will need to have a full driving licence and access to a vehicle that can be used for work purposes.
You must be a registered healthcare professional (NMC) as you will be administering Intravenous medications.

* Assessment and review of treatments.
* Support patients through their treatment via agreed clinical pathways.
* Review care plans and escalate concerns appropriately.
* Deputise for the virtual ward manager in their absence.
* Any other duties associated with the Hospital @ Home service.
Hospital@Home operates across all three sites within the Mid & South Essex University Hospitals Group and you will need to have a driving license and access to a car in order to perform this role. Work travel expenses are paid.
