A nationally recognised, innovative nurse-led clinical team have an opportunity for an experienced and dynamic individual to join the team as Clinical Nurse Specialist.
Outpatient Parental Anti-microbial Therapy (OPAT) is a cross site service providing Intravenous Antibiotic treatment for patients within a domiciliary and clinic setting including patients who are trained to self-administer medication.
The successful candidate will be responsible for providing nursing expertise, effective management of a defined patient caseload and management co-ordination of care with liaison with the MDT. They will provide on patient counselling and education from initial referral through to discharge and end of treatment. The successful candidate will provide a pivotal link between in-patients and the domiciliary team, attending MDT meetings, deputising for the Lead Nurse and completing highly specialised clinical duties.
It is crucial that the successful candidate contributes to quality improvement and developing the speciality service, supporting opportunities to expand and grow the service.
• Deliver high quality patient care as an Clinical Nurse Specialist to a defined caseload of patients covering a 7 day service
• Assess, develop and implement specialist nursing care programmes and evaluate care and treatments
• Provide highly specialised advice and treatment plans contributing to the individual care of patients
• Plan, provide and facilitate specialist training and education of multi-professional staff, learners, patients within the speciality
• Escalate patient safety concerns and contribute to learning from incidents
• Provide cross site cover throughout the Trust according to service demand, in clinic settings and in patient's homes. This will require the successful candidate to have access to a vehicle and a clean full UK driving license
