We’re looking for a Weldmar Community Nurse for our newly formed Specialist Hospice at Home Service in North Dorset.
Please note: there are two posts available for this role – 1 for 30 hours per week and 1 for 22.5 hours per week.
You’ll be joining Weldmar at an exciting time, working collaboratively with the multi-skilled team in our South Hub, other clinical colleagues and external agencies to provide an agile and responsive service based on patient needs in Sturminster Newton, Sherborne, Blandford, Shaftesbury and surrounding areas.
This new and exciting service will see our patients, carers and families benefiting from receiving care most appropriate to them, as determined on a daily basis. They will have access to a range of support from those working within the hub, other clinical specialists from our multi-disciplinary team, and to our patient and family support services, as well as to hands-on care if required to enable them to live as well as possible at home.
You’ll use your significant post-registration experience as a registered nurse to complete ongoing assessments of our patients to ensure the delivery of quality holistic evidence-based palliative care. You’ll have the time to assess the complex needs of our patients and their loved ones, and to take steps which will make a positive difference to them when it matters most.
You will also play a key role in providing advice, coaching, and guidance to your colleagues within the hub, as well as to other health professionals ensuring the delivery of outstanding end-of-life care in the community.
Your core hours will be Monday to Friday between 9 am and 5 pm, with a requirement to provide out-of-hours weekend cover for the advice line on a rota basis.

Clinical Salary Scale:
£37,338 - £44,962 per year FTE (pay award pending)
