Independently manage a group of patients within the speciality whilst working as part of the multi-disciplinary team, delivering specialist individualised and personalised direct patient care. This includes history taking, physical examination, and the requesting of clinical care management changes. Be responsible for assessing and recognising emergencies in the specialty. To be an effective communicator with patients/those important to the person/families regarding treatment plans as well as symptom management. To promote high quality clinical care and assist in the development of the specialist service, through use of evidence based practice and clinical standards. Provide a seamless, high quality service from referral through to assessment, treatment and review. Ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines. The promotion of best practice in the care of people at their end of life with the aim of enhancing the patients and those important to the persons experience through evidence based practice. Assist the Tissue Viability Specialist Lead to maintain and improve current pathways. Participate in Audit as required. Audits require evaluating the impact of Local and National strategies, staff training and awareness. To assist others with the continued evaluation of the service ensuring key National and organisational guidance are incorporated and embedded. Interpret information and take appropriate action whilst leading others to do the same. Lead on and participate in key developments relating to specialty so that high quality patient focused services are promoted. Attend and participate in multi-professional team meetings/steering groups. Organise own work and manage own caseload and practice, identifying and maintaining supportive networks for self and other staff members.
