Qualifications
Essential criteria
* Registration with NMC
* Recognised teaching/mentor qualification
Desirable criteria
* Non-medical prescriber V300
* Documented evidence of having undertaken relevant further study or training appropriate to the role of Case Manager
* Degree/diploma in Clinical Nursing or evidence of working towards
* Leadership/management qualification
Experience
Essential criteria
* Significant post-registration experience
* Demonstrate a working knowledge of community nursing, case management, and management of a patient within a community setting
Desirable criteria
* Experience of working within other relevant areas of healthcare
Skills/Knowledge
Essential criteria
* Excellent communication skills (written & verbal)
* Effective time management
* Assertive
* Change manager
* Good Health and Social Assessment Skills
* Knowledge of current Health and Social Care Legislation affecting people with Long Term Conditions
