North Colchester PCN (Primary Care Network) have an exciting opportunity for a care coordinator to join us in assisting to support primary care communications across our PCN. This position is offered on a 18 month fixed term contract for 20 hours per week. There is the possibility of this becoming a permanent position after this time. The role involves working very closely with the PCN business manager and clinical director. You will also work with the multi- disciplinary team (MDT) within the PCN. The role is pivotal in ensuring the smooth running of the PCN. Collate and develop promotional material to improve uptake of existing and new technology. Support the PCN team with any relevant projects and tasks. Schedule meetings and manage meeting minutes. Assisting with HR administration. General administrative tasks in order to support across the PCN. About the Candidate If you are an excellent person-centred individual with co-ordination experience, with proven admin and communication skills with a keen interest to support and work as part of a team to make big changes, then we'd love to hear from you. The ideal candidate will hold the following experience and key attributes: Experience of working within healthcare or the voluntary/community sector, supporting vulnerable groups. Ideally you will have a Diploma Level 2 in Health and Social Care or equivalent qualification. You will have strong listening and communication skills. You will be familiar with local resources and services and how to access them. You will be motivated by helping people, with care and empathy and in delivering person-centred support in a non-judgmental way. Strong administrative experience. Proficient in Microsoft Office.
