An exciting opportunity has arisen for an Advanced Clinical Specialist Falls Lead within community.
The role is a 12 month fixed term position, involving working on an ongoing quality improvement project, targeting patients who have fallen or are at risk of falling, but currently do not meet referral criteria for services such as community rehabilitation and therefore, would not routinely receive a multifactorial falls assessment.
Hours per week: 30 hours
Main duties of the job
The role will work collaboratively with the falls lead in Mid Cheshire to continue project based work, commenced by the existing post holder. The role will also offer Advanced Clinical guidance to care community therapists across East Cheshire and will work closely with multi disciplinary team colleagues to offer falls training and support to care homes across East Cheshire to improve quality care.
Upon completion of the project over the next 6 months, the role will then expand and embed the new falls pathways across East Cheshire Care communities. The role will adopt a more clinical focus with a continual drive for service development and quality improvement.

* HCPC (Health and Care Professions Council) registration
* Relevant professional degree
* Master's degree level or equivalent experience
* Professional/clinical knowledge acquired through degree supplemented by specialist training experience in specialist fields and practical experience to postgraduate diploma level or equivalent
* Membership of a relevant professional body & related specialist sub-groups

* Post registration knowledge which is relevant to the specialist area
* In-depth knowledge of recent NHS legislation, recommendations for specialist area
