Job summary We have an exciting opportunity for a Band 6 to work within Longmoor House. Mersey Care NHS Foundation Trust and Liverpool University Hospitals NHS Foundation Trust have worked in partnership to develop a brand new physical health service to improve patient outcomes. Longmoor House is a newly refurbished Integrated Care Unit that provides step down capacity from acute trusts for medically optimised patients where it is anticipated that there are ongoing nursing, therapy and reablement needs. Supported by senior practitioners, they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes. Align role to deliver care according to national service frameworks and tackling and preventing health inequalities. Support the multi-agency team on the ground with emphasis on a sound communication network.
