The ideal candidate should have a good understanding of Primary Care, knowledge of Chronic Disease Management, Immunisation, and Cytology experience or be willing to undertake any necessary training required for the role. They should have an understanding of QoF and be committed to providing excellent care to our patient population.
Main duties of the job
To work as a team with the Partners, Practice Nurse, and administration staff to provide skilled and effective care to the practice population.
To deliver care in accordance with the agreed Practice protocols and objectives.
To assess, plan, deliver, and evaluate care holistically to the practice population.
Delivery of Chronic Disease management.
Clinical Support on the management and care of patients with Long Term Conditions.
Plan, implement, and evaluate individual treatment plans within the remit of your own role.

We are a 3 Partner, 3 Salaried GP, 3 ANP, and 3 PN team supported by 2 HCAs and 1 TNA and a full Management and Admin team. We are situated next door to Sunderland Hospital in a purpose-built building. We work together to provide the best service possible to our 10,300 patients from across the city.
Job responsibilities
Undertake annual reviews of patients with Chronic disease and long-term conditions and ensure full QoF and QP achievement.
Carry out annual and interim management of patients, in line with Local & National Guidelines for:
1. COPD
2. CKD
3. Asthma
4. Chronic Heart Disease
5. Diabetes
6. Immunisation Clinics
7. Cervical Smear and Sexual Health
8. Venepuncture/ECG/Basic Dressing
Person Specification
Qualifications
* RGN Qualification
* Current NMC registration
* Accredited training in chronic disease management
* Cervical cytology qualification
* Up-to-date training in childhood immunisations
* 2 years post-registration experience as Practice Nurse
* Health promotion
* Family planning
* Evidence of recent updates in primary care
* Working in General Practice/Primary Care
* Working with Clinical system
