We are looking for experienced community Healthcare Assistants who have the skills, knowledge, and experience to assist the nursing team in delivering excellent patient care. Main duties of the job
This is a hands-on patient-facing role; we are seeking individuals to join our team who are caring, compassionate, and confident, who demonstrate effective communication, excellent customer service skills, good IT skills, and the ability to support patients with interventions and/or queries. The postholder will be a key member of the practice team, responsible for undertaking patient health checks including BP / ECG monitoring / height and weight, taking blood samples, health and wellbeing advice, and promotion.
Job Description

* Communicate courteously and effectively with patients, relatives, and carers.
* Carry out health assessments on patients e.g. testing urine, blood pressure checks, heights, weight, and taking simple document medical history.
* Chaperoning patients during clinics.
* Report all matters pertaining to patient care – changes in condition/circumstances to a relevant member of the nursing or medical team as a matter of priority and ensure that a documented record is kept.
* Ensure that the clinical rooms are kept in a clean and orderly manner and that relevant equipment is ready for use by nursing and medical staff during clinic sessions.
* Assist in clinic sessions as directed by members of the clinical team ensuring that all equipment is clean and sterilised following clinic sessions.
* Ensure that the drug refrigerator is at the correct temperature on a daily basis and that drugs are within their expiry dates and used in the correct sequence. Order drugs and other supplies as directed ensuring that adequate levels of stock are available in the surgery. Record fridge temperatures.
* Undertake phlebotomy as directed.
* Act at all times with regard to the confidentiality and sensitivity of issues to do with patients, their care, and treatment.
* Record and document all patient contact.
* Carry out relevant clerical and reception duties as required.
* Assist in audits as required e.g. note sampling, data input, and information gathering.
* Attend relevant meetings as required.
* Have knowledge of and act at all times within the boundaries of Health and Safety legislation and data protection acts.
* Comply with all service policies and procedures and health, safety, and fire regulations.

Education
Evidence of Higher education pathway (NVQ level 2/3 in Primary Care).
Evidence of Continuing Professional Development.

Minimum 1 year’s experience in Health Care Assistant Role.
