We are looking to recruit a community matron to ideally work over three days per week approximately 20 hours per week.
You will work with patients to promote shared decision making regarding their care plans, encourage them to make informed decisions about their health, and to self-manage long-term conditions with the aim to prevent hospital admissions.
Main duties of the job
1. Current NMC registration
2. First Level relevant Degree or Equivalent
3. Non-Medical Prescriber
4. Evidence of master's level study
5. Demonstrates a high level of relevant clinical experience.
6. Experience of working with people with complex health needs
7. Able to carry out physical examination and comprehensive health assessment.
8. Demonstrate competence in safe medicines prescribing.

12. Evidence of knowledge and experience of case management

About us
Matlock and Ashover Practice is made up of two surgeries: one at Imperial Road Surgery in Matlock with 7600 patients and a second Surgery in the village of Ashover with 2400 patients. We have five GP partners and a further five salaried GPs. We are also a training practice with both registrars and medical students. Our primary aim is to serve the General Public with first-class medical services.
