An exciting opportunity has arisen for a new role as a Band 6 Triage Nurse. The triage nurse will assess new referrals or messages to determine when and if the patient requires a visit. You will act as an autonomous practitioner providing a comprehensive telephone triage and clinical assessment service to patients and/or their carers/relatives, resulting in a safe and appropriate clinical decision.
The position will be within The East Locality Hub following a transformation of the community nursing service.
Main Duties of the Job
The clinical triage role will be a fundamental part of the new teams, supporting the clinical triage of new patient referrals and messages. These new referrals come through Single Point of Access (SPA) from the following services: GP, Hospital, Health Care Professionals, self-referral, and social workers. The clinical triage role is to triage the referral/message against the IMT specification to determine if the patient meets the criteria for a Community Nurse Team visit. The role will also involve triaging messages from patients on the current caseload to establish if a visit is needed and the urgency of the visit. The post holder will also support in the rescheduling of patient visits and the review of the caseload for patient scheduling.
