You will lead on all aspects of medicines optimisation and deprescribing within the elderly care speciality, supporting medical and nursing teams, as well as allied health professionals in the safe and effective use of medicines. Main duties of the job Providing a clinical pharmacy service to the elderly care wards including ward round attendance to support medicines optimisation and deprescribing initiates for the polypharmacy population Training and development of rotational junior and trainee pharmacists in elderly care.
