Trinity Hospice has been at the heart of the Fylde coast for 40 years, providing compassionate care for people on their journey towards the end of life. Our dedicated specialist nursing teams touch the lives of thousands of patients and families each year, with one vision for the future – that everyone across Blackpool and the Fylde coast has access to high quality end of life care.
We are looking to recruit an experienced, highly motivated Senior Occupational Therapist to join our Living Well Service to provide a high quality, innovative Occupational Therapy service for patients with palliative care needs.
You will need to be qualified at degree level and have relevant post registration experience at a senior position.
You will work autonomously as an advocate of the rehabilitative approach to care to deliver high quality Specialist Occupational Therapy interventions, managing a caseload of patients with a wide range of life-limiting conditions to mutually agree goals, monitor, evaluate and modify treatment using appropriate outcome measures to progress and ensure effectiveness of interventions.
You will also provide support and advice for patients’ families, friends, and carers as appropriate and within your scope of practice whilst adhering to HCPC and the Royal College of Occupational Therapy standards.
As well as outpatient, group, patient homes, and community-based work, there will be in-reach to the adult inpatient unit to support rehabilitation and discharge planning for hospice patients and involvement in supporting the wider Trinity strategic development of services.
