Clinical Responsibility The Nurse Assessor will need to lead and document Continuing Healthcare assessments and/or Funded Nursing Care assessments. This will include frequent requirement for intense periods of concentration - please note that assessments are often conducted in the service users own home, or in care home settings where there can be an increased level of noise and distraction. The Nurse Assessor will be required to: Complete CHC or FNC assessments that are clear, concise and detail plans to safely manage any risks that are identified. Use a variety of clinical skills appropriate to the needs of the individual and the clinical setting in which they are seen. Demonstrate a high standard of record keeping and documentation with adherence to Trust policies Work with colleagues to develop and demonstrate clinical expertise and high standards of mental health practice. Promote care that is person centred, reflecting current best practice, and challenges practice that may be detrimental to older people, which may include the need to raise safeguarding concerns when appropriate. Demonstrate extensive understanding of the legal and ethical issues in providing mental care. In particular having a good knowledge of the Mental Health Act, the Mental Capacity Act, The Care Act and the National Framework for Continuing Healthcare. Communications The successful applicant will: Have a high level of competency to receive and give clinical information verbally and in writing from/to those receiving care, their families/representative, to professional colleagues from health and social care, and to outside agencies. Demonstrate a high level of interpersonal skills, self-awareness and empathy in all communications. The assessments can sometimes involve discussing information that the service user or representative may find upsetting or distressing. Ensure adherence to all aspects of patient confidentiality, documentation and record keeping according to Trust and national guidelines. Will have the ability to communicate clearly and sensitively in complex and emotional situations. Will have an ability to negotiate and problem solve in contentious and/or antagonistic situations where agreement is required. When agreement cannot be reached, the successful applicant will be required to communicate clearly and rationally to justify recommendations they are making, whilst ensuring that others views are taken into account. Will have the ability to develop and maintain good working relationships with colleagues within HPFT and all agencies who they encounter to the benefit of the service users and the service. Other Develop professional practices, skills and competencies within continuing healthcare. This may also include sharing knowledge and skills with fellow health and social care colleagues. Develop and maintain professional links with colleagues throughout the Trust. Be aware of and to comply with NMC codes and guidelines. Produce and maintain evidence of NMC registration. Partake in clinical and management supervision. Engage in team meetings, and ad hoc discussion meetings relating to the role.
