Our Short Breaks service based in the Grindon Area of Sunderland are seeking to recruit passionate Support Workers who can deliver exceptional standards of person-centred care and support to meet various support needs of individuals, which may include physical disabilities, learning disabilities, autism and/ or mental health needs.
You will be part of a team supporting customers with personal or health care tasks, administering medication, money management, access community activities or provide emotional and/or behavioural support whilst developing and maintaining positive relationships with their families and carers. In addition, you will co-ordinate and contribute to the ongoing development of care plans, day to day quality and risk management, ensuring the seamless transition from home to short breaks ensuring person-centred care and support is given to customers and their families.
