Job summary Our Community GP's have the opportunity to apply their experience, knowledge and interest of Highly complex patient care in the patients own home through working with the Team Up Home Visiting Service in Swadlincote. You will clinically lead a team of highly skilled and knowledgeable Advanced Clinicians to deliver complex holistic patient care. You will also support the Service Clinical Leads and Operational Lead in developing and applying service improvements. You will also provide support to our trainees, helping them achieve their academic goals. Mentorship, guidance and support to clinical decision making throughout the whole extended team. Medical assessment and management on behalf of and in conjunction with members of the team. Senior clinical triage for the Acute Home Visiting service, to determine the urgency and type of response needed, according to clinical need. Provide education and mentorship to members of the team, especially those in advanced or extended practice roles, including trainees.
