An exciting opportunity has arisen for an Assistant Practitioner to join Adult Community Therapy around the Spalding area. As a therapy-focused Assistant Practitioner, you will complete assessments with people in transitional care beds within care home settings to coordinate the care of patients who have been admitted into these beds while working alongside registered therapists and therapy assistants to ensure their rehab and holistic needs are being met. You may also be asked to assess patients in their own homes to maximize their levels of independence. To apply for this position, you must have a foundation degree or equivalent.
This post is likely to commence from April 2025.
Main Duties of the Job
This position is to support patients who have been admitted into transitional care beds within a care home setting or visiting patients in their own homes to address any therapy needs. You will need to deliver high-quality patient-centered care while working within a multidisciplinary team (MDT) and therefore have good communication skills. You will hold a caseload of patients and complete holistic assessments of the patients who come into our care and support admission and discharge plans as well as identification of any therapy needs. Therapy experience is preferred but not essential. You will need to have an ability to travel, possibly around the county, but more locally around Spalding and Holbeach areas due to the community nature of this position.
