The role is central to the coordination of both complex and non-complex discharges. The post will be based within the Patient Flow Hub and have responsibility for overseeing non-complex and complex patient discharges within PTHB Community Hospitals. In the course of their duties, the post holder will be responsible for:
Facilitating and/or assisting with effective, efficient, and timely discharge of patients from the hospital. Particular emphasis is placed upon quick turnaround of patients. This may include complex multi-disciplinary assessment signposting, liaising directly with patients and their families/Care Transfer Coordinators/MDTs, assisting in case conferences for patients with complex health and social care, palliative care, or rehabilitation or housing needs. Giving families support and information.
Support the patient flow hub, as well as working closely with a range of professionals and external agencies within the hospital and in community services.
Work independently, following procedures and treatment plans, but under the direction of the patient flow hub, and within agreed boundaries.
Responsible for ongoing/complex planning regarding process mapping patients' journeys for modeling purposes.
Provide training on systems to new staff joining the team.

This may include complex multi-disciplinary assessment signposting, liaising directly with patients and their families/Care Transfer Coordinators/MDTs, assisting in case conferences for patients with complex health and social care, palliative care, or rehabilitation or housing needs. Giving families support and information. Support the patient flow hub, as well as working closely with a range of professionals and external agencies within the hospital and in community services. Work independently, following procedures and treatment plans, but under the direction of the patient flow hub, within agreed boundaries. Work as part of the flow hub and work effectively with the wider multi-disciplinary team and third sector groups to facilitate patient discharges in a timely manner across Powys. Supporting ward staff and multi-disciplinary teams in facilitating effective, safe, and timely patient discharge. Providing the patient flow hub with updated information on patients requiring discharge.

Qualifications
* Experience of working in an administrative capacity within Health or Social Care teams at NVQ Level 3 level or equivalent
* Knowledgeable about discharge planning from a Health or Social Care setting
Experience
* Experience of work in an administrative capacity involving a range of disciplines. Administrative skills including excellent IT skills. Experience of multidisciplinary team working. Understands the need for strict confidentiality.
* Previous experience of NHS or Social Care clinical setting. Working in a high pace role and adapting to change depending on workload.
