Working as a Health Care Assistant for Leeds Community Healthcare Trust in a secure environment is very different from anything you will have done before; we work as a community within a community. The Wharfedale Recovery Hub Bilberry and Heather units is located in Wharfedale hospital 2nd floor, Newall Car Road LS21 2LY Otley and is for patients who require a short stay for up to 6 weeks after being discharged from hospital. Home first is the ideal outcome for many patients leaving hospital, but a temporary stay in a homely environment that can provide 24-hour care is sometimes required. Wharfedale recovery hubs provide rehabilitation and plan for their safe discharge to either a nursing/care home or to their own home. Our aim is to work with our patients and help them achieve their recovery goals with the support of our nursing and therapy team, which includes Occupational Therapists, Speech and language Therapists, Physiotherapists, Dieticians, Registered nurses, GP and Health Care support workers.
The hub operates 24 hours a day, 365 days a year and you will support the patients on various shift patterns to maintain this operation.
Main duties of the job
The post holder will be providing direct and indirect patient care independently but with support available from a supervisor within the clinical area. In doing so, they will support the assessment of patients and implementation of programmes of care delegated to them by the appropriate staff member ensuring that they practice within their sphere of competence and knowledge. The post holder will work within a range of procedures and pathways in which they have received training to be competent.
* Assisting with daily living activities such as eating, showering and using the toilet
* Utilising equipment to move patients when necessary
* Helping tidy the patients' room
* Taking the patients' vital signs such as blood pressure and temperature
* Talking to patients and reassuring them
* Attending meetings with other healthcare professionals
* Assisting nurses and other staff when needed

Job Purpose
The post holder will be providing direct and indirect patient care with the support and supervision available from the staff who are responsible for the care of patients within the clinical area. In doing so, they will support the assessment of patients and implementation of programmes of care delegated to them by the appropriate staff member ensuring that they practice within their sphere of competence and knowledge. The post holder will work within a range of procedures and pathways in which they have received training in order to be competent.
Key Responsibilities
1. Clinical
1. The post holder will deliver care to patients within the limits of the procedures and pathways in which they have achieved competence.
2. Support and assist other staff members to deliver care as delegated and directed.
3. Demonstrates competent physical skills obtained through practice e.g. use of moving and handling equipment, routine tests within agreed pathways of care.
4. Able to identify risk or changes in condition and are responsible for taking action and alerting the appropriate staff member.
5. Exchanges factual information with patients using persuasion, reassurance, tact and empathy and addressing barriers to understanding in order to engage the individual in their care and gain valid consent; records discussions accurately and in a timely manner in the individual's patient record.
6. Engages and actively involves the individual and, if appropriate, their family/carer, in their programme of treatment and/or care, if directed to do so by the responsible professional, by using personalisation techniques to promote a culture of self-care, involvement and empowerment.
7. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role.
8. Prioritises their own workload with supervision of registered and/or responsible staff and understands the boundaries of their practice.
