The Later Life service in St Helens works with individuals aged 67+ with age-related frailty and mental health issues such as dementia. The team consists of 15 Social Workers and 15 Assessment and Review workers. This is a busy team with the Social Workers completing Care Act assessments, devising care plans, and reviewing services. The Social Workers have a busy caseload and are expected to manage a high number of referrals. It is expected that the Social Workers will have a good understanding of current legislation and government policy related to the care of the elderly, as well as an understanding of the Mental Capacity Act and DoLS legislation. This post will predominantly involve completing hospital discharges for out-of-borough hospitals, which will entail visiting these hospitals to complete Care Act assessments, Mental Capacity assessments, best interest assessments, and attending case conferences as required. Good support structures are in place, and regular supervision is provided by team managers or advanced practitioners. The Later Life team works on an agile basis; therefore, a car driver is essential. The working hours are Monday – Thursday 08:45 – 17:15 and Friday 08:45 – 17:00. We are unable to offer condensed hours, although we do offer some flexibility.
You must have:
* A professional qualification in social work
* Social Work England registration
* Post-qualified experience
* Enhanced DBS/Update service
* References required for the last 5 years of employment
