We are excited to offer bank opportunities for experienced Registered General Nurses to work within our Care Community Teams.
We can offer various hours across our teams. If you are enthusiastic, motivated and keen to support and deliver a range of quality nursing services to our patients within the locality of Western Cheshire then we'd love to hear from you.
Main duties of the job
As a Community Nurse the successful candidates will undertake duties involving the delivery of nursing services and the holistic assessments of patients' health needs using a person centred approach.
You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long term care settings.
An enthusiasm to support our teams to deliver person centred care is a must for this role. You will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable and approachable.
