Falls Prevention Therapy Lead - Community
Join to apply for the Falls Prevention Therapy Lead - Community role at Torbay and South Devon NHS Foundation Trust.
Working as part of the Trust Falls Prevention Team, you will deliver falls prevention across the community and partner agencies (e.g. SWASFT, primary care, care homes, and care agencies) for adult fallers, implementing all aspects of best practice to prevent falls and reduce the impact of harm and injury when a fall occurs. You will link policies and pathways across the Torbay and South Devon NHS Foundation Trust (TSDFT) to enhance integrated care.
Responsibilities include:
1. Work alongside clinical teams, patient representatives, and external partners to support them with expert falls knowledge, lead falls initiatives, and progress work in line with national initiatives, guidance, improvement plans, and learning from incidents.
2. Act as a highly visible and proactive leader, ensuring staff are competent, trained, and up to date with the most current falls prevention evidence base.
3. Advise on relevant policies and workstreams where complex issues related to falls prevention are involved, supporting community staff with specific falls problems in patients with complex presentations.
4. Provide education on falls prevention across TSDFT, specifically for individuals aged 65 years and above, as well as those aged 50 plus with long-term conditions.
5. Co-lead on national audits relating to falls prevention and maintain the database of falls data, reporting to Falls Improvement Steering Group (FISG) governance and relevant meetings.
6. Support and co-chair bi-monthly FISG meetings and disseminate outcomes via minutes, action logs, and falls newsletters.
7. Review the falls dashboard (Tableau) each month to analyze trends.
8. Link with the Quality Assurance Team regarding care home residents with complex presentations and challenging falls prevention issues.
9. Co-manage the postural stability strength and balance service in conjunction with the Public Health and Personalised Care Manager.
