Job summary The Community Ready Unit (CRU) (previously Discharge Lounge) has been developed to provide a relaxed, safe & comfortable environment for patients awaiting discharge or transfer to another care facility allowing hospital wards & departments to free up acute ward based beds as early as possible to support patient flow. The CRU is a busy, small and friendly unit with a multidisciplinary team comprised of Nurses, HCSWs, Ward Clerk, Housekeeper, Pharmacy Technician & Domestic staff and also has an on-site a Doctor during weekdays. As a Registered Nurse working within the CRU you will be responsible for the coordination of the unit, supervision of junior staff & the delivery of direct patient care. Other duties include moving and handling of patients and equipment as may be necessary, care of patients nursed in bed, assistance with mobility, supporting hygiene needs & promoting the privacy & dignity of patients. Also included are diet and fluids, administration of prescribed medications & IV drugs, point of care testing, minor dressings & removal of IV cannulas, checking TTOs & health education. The post holder will also be required to liaise with colleagues across the Trust, including the wider Corporate Operations and Site Management Team, Discharge Coordinators, Hospital Pharmacy, Integrated Discharge Team, Hospital Transport and ward and departmental areas. The role will also involve liaison with Patients families, District Nursing Teams, Care Homes social care providers. Main duties of the job Operational and Clinical Coordination of the Community Ready Unit. Providing personal care for patients. Assisting with the transfer and movement of patients both from ward areas and within the Community Ready Unit Providing patients with prescribed medication. Checking TTOs and Discharge Letters. Confirming patients home care and care homes who are expecting them. Liaising with District Nurses to ensure care is booked. Booking transport for patients. Discussing discharge expectations and arrangements with both the patient and families, while answering any questions. Assessing patients needs, to ensure they leave the Hospital with the correct level of care.
