Job summary
The Care Coordinator is an essential member of the Nottinghamshire CYP Weight Management Service, contributing to the effective coordination and improvement of care for children and young people (CYP) within the weight management programme. Working within a specialised team, the Care Coordinator ensures seamless communication, collaboration, and support across various healthcare professionals and stakeholders.
Main duties of the job
You will provide the lead role on administration and engagement co-ordination for all referrals into the Single Point of Access service. You will actively develop engagement activities and ensure the smooth running of the administration of the referrals and MDT meeting to meet the targets and deadlines.
The Care Coordinator will be required to review data and offer valuable insights to drive project success. Your analytical skills will be crucial as you assess data, extract meaningful insights, and present findings to the leadership team, aiding informed decision-making processes. Additionally, you will play a role in representing the service at PRB (Project Review Board) meetings.
An essential part of the role is to manage the weekly MDT, coordinate the weekly meetings, send invites to attendees, gather assessments, and as much other information as possible from professionals involved in the individuals' care during the meeting. The Care Coordinator will monitor, report and analyse attendance, and completion of actions assigned to the attendees.
You will join a large team of behaviour change specialists in ABL Health including specialist lifestyle coaches, nutritionists, dietitians, GPs, psychotherapists, and exercise specialists to deliver evidence-based, holistic and realistic weight management to people in the community.

Person Specification
Qualifications Essential
* A bachelor's degree or equivalent in a relevant healthcare or social sciences field
* Administration based qualifications
* Proven experience working in a healthcare or social services environment, preferably with children and young people
* Previous involvement in leading care coordination, case management, or similar roles is advantageous.
* Familiarity with multidisciplinary team collaboration and patient-centred care models.
Desirable
* Additional certifications or training in childhood obesity management, care coordination, or related areas are desirable
Experience Essential
* Be an experienced administrator used to working in a health and/or community setting
* Experienced working with Microsoft Office and health-related database systems
* Positive and proactive, working at pace with multiple tasks
* Adept in communication with a positive and welcoming style towards all patients, health professionals, and colleagues
* Build strong relationships, good team player happy to work as part of a team and work independently on own tasks
* Quality-driven, you naturally seek high standards and actively seek to improve them.
